As a Community Support Worker (CSW) you will be working within the Intensive Community Care pathway and Urgent Community Response team covering Oxfordshire county. Working independently and under direction and support of a registered clinical practitioner, the CSW will visit people at home who are experiencing an urgent health and social care crisis. The CSW helps the person in their immediate recovery and aims to enable them to remain at home or place of residence (where safe to do so) without the need for an unnecessary hospital conveyance. The CTS role is suited for someone who is compassionate, resilient and motivated to make a real difference. Community urgent same day care services operate county wide and the role will be working shift patterns and occasional weekends between 8am and 8pm, 365 days a year. The post-holder may be required to work flexibly across shift patterns and will operate within a locality area; however, the services are county wide and some travel will be required. Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Main duties of the job
- Deliver care to people within their own homes by supporting registered and unregistered colleagues in the delivery of safe and effective patient/client care and management plans and other duties under guidance and supervision.
- Work collaboratively with colleagues, carers and partner agencies to promote continuity of care and effective communication across services and organisations.
- To establish and maintain professional working relationships within urgent community response, community response, South Central Ambulance Service (SCAS), Primary Care and with patients/clients, carers and relatives.
- Accepting and relaying information and messages as required and reporting any changes in condition to a registered practitioner. This may include staff from other agencies or localities when a patient/client’s care is being transferred.

Detailed job description and main responsibilities
- To provide hands on care, planned advice, support, and instruction to patients and where relevant their carers and other professionals, promoting understanding of the aims of their treatment plan / goals and to ensure a consistent approach to patient care.
- To communicate effectively with all patients including those with mental health needs, and their families/carers using a range of verbal and non-verbal communication tools to progress agreed management plans and treatment programmes ensuring patient understanding of treatment proposals to gain valid informed consent according to organisational policies.
- To evaluate emergency situations and act accordingly working within procedures and protocols laid down including knowledge of DNACPR protocols.
- Carry out duties across community settings including lone working and visiting service users in their own homes travelling independently between sites as required.

Qualifications
Essential criteria
- Level 2 or 3 Diploma in Health and Social Care.
- Evidence of self-directed learning.
Desirable criteria
- Advanced qualification beyond Level 3
Knowledge and Experience
Essential criteria
- Previous experience as a care worker/formal carer with adults.
- Experience in Phlebotomy and taking observations
- Experience of working autonomously within a community setting without direct supervision.
Desirable criteria
- Experience working within a community health care team(s)
- Experience of receiving clinical supervision.
People Skills

- Well-developed communication and interpersonal skills.
- Able to work as a member of a team.
