Community Health Care Coordinator

Hartlepool Network PCN is seeking a Health Care Co-ordinator to support proactive, personalised care and MDT working across primary and community services. The role involves disease management, care navigation, and coordination under supervision of registered clinicians.

Ideal candidates are Nursing Associates or experienced Health Care Assistants with the skills to deliver DES/PCN aligned care, including planning, risk stratification and patient-centred support within defined frameworks.
