We are seeking an exceptional individual to join our Dermatology Skin Cancer team as a Macmillan Cancer Care Co‑ordinator, delivering best‑practice personalised care and support for people living with and beyond skin cancer as part of our Living with and Beyond Cancer (LWBC) Programme.
In this role you will work closely with the Skin Cancer Clinical Nurse Specialists (CNS), supporting patients and their families throughout their cancer pathway, helping them navigate services and manage the practical, emotional and psychosocial impact of a skin cancer diagnosis and treatment.
You will be part of the Trust Cancer Care Co‑ordinator teams, embedding and integrating personalised care across skin cancer pathways, ensuring that support is accessible, timely and tailored to individual need. You will also work with the wider LWBC team to support the planning and delivery of Health and Well‑being events and other personalised care interventions.
We are looking for someone who can work autonomously, confidently make decisions in a variety of situations, communicate effectively and work well as part of a multidisciplinary team.
Main duties of the job
The Macmillan Cancer Care Co‑ordinator will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer and their carers throughout the care treatment pathway and after care support. The post holder will work in close unison with the Clinical Nurse Specialists to ensure the coordination and management of patients at all points of their treatment. The Co‑ordinator will also act as a point of contact for patients prepared by their clinical team for Patient Triggered Follow‑Up (PTFU) and will be able to sign post patients to support their self‑management.
The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists.
Qualifications
- GCSE English language and Mathematics (Grades A‑C) or equivalent
- Recognised ECDL or equivalent or extensive experience using Microsoft Office package
- Communication skills training (e.g., Thyme and Sage)
Experience
- Experience of working in the NHS in a patient‑facing role
- Experience of multi‑professional working
- Experience in data management
- Experience coaching/teaching patients and carers
- Experience in Cancer Care
- Understanding of Macmillan Cancer Support and its role across the UK
- Awareness of Improving Outcomes: A Strategy for Cancer and other national cancer policies
