We are currently seeking a Band 6 Senior Community Nurse/District Nurse to join our dedicated team, working across the Acton and Ealing localities. This is an excellent opportunity to be at the forefront of community healthcare, leading innovative initiatives and making a real difference in patients’ lives.
Key Responsibilities
- Admission Avoidance: Implementing proactive care strategies, such as participating in MDTs, caseload reviews, and complex patient reviews/visits, to prevent unnecessary hospital admissions.
- Health and Independence Promotion: Empowering patients to manage their health conditions effectively at home, utilising personalised care plans and supporting the NHS’s commitment to enhancing out-of-hospital care.
- Leadership and Development: Supporting in leading a dedicated community nursing team within a multi-professional environment, fostering a culture of continuous learning and improvement.
- Practice as a senior member of the community nursing team, working within the Integrated Community Services (ICS).
- Provide a case management approach to care for an identified number of patients working closely with GPs, the ICs and other statutory and voluntary agencies.
- Provide advice, information and education to patients and carers with the aim of enabling patients to enjoy maximum independence, health and wellbeing.
- Creatively manage the holistic and clinical needs of patients with acute, sub-acute and long-term conditions.
- Enable patients living long term physical health challenges to live well within their communities, with a focus on not only their physical health needs but also their wider holistic needs, such as mental health, learning disabilities, psychological needs, social needs and spiritual needs.
