Team Leader (Supported Living) – Huddersfield, West Yorkshire, England
Radis Community Care is looking for a Senior Support Worker/ Team Leader to lead our Supported Living services in Huddersfield and Halifax. The role involves providing person‑centred support, overseeing daily operations, ensuring service quality, and developing individualized care plans.
Key responsibilities include:
- Managing and scheduling support workers, coordinating shifts, and covering gaps.
- Providing direct support with medication, personal care tasks, and medication administration.
- Conducting regular audits, checks, supervisions and spot checks.
- Updating and reviewing support plans to help individuals achieve their outcomes.
- Maintaining a consistent approach across the life‑cycle of service users.
- Working closely with Managers of the Supported Living services.
- Ensuring office duties and alternate weekend work are completed (office based in Bradford).
- Using own vehicle for local travel as required.
Required skills and qualifications:
- Previous experience working with individuals with learning disabilities, autism, and mental health issues.
- Experience as a Team Leader (essential).
- NVQ Level 2 in Health and Social Care, currently working towards Level 3.
- Good written and verbal communication skills.
- Willingness to learn in line with client needs and expectations.
- Enhanced DBS check required.
- Valid driving licence and own vehicle to cover localities.
