Discharge Flow Coordinator

Whittington Health is seeking a Patient Flow Coordinator to lead discharge planning for clinically ready patients, ensuring timely transfers and safe ward processes. The post holder will work closely with MDT and bed/site teams to promote proactive discharge planning and maintain high standards of care.

You will coordinate with families, clinical teams, and external services to reduce delays, support complex discharges, and deliver patient-centred care aligned with Trust policies.
