As a Community Support Worker in the Therapy Team, you will support patients to live independently and improve their quality of life in the home environment. You will work closely with multidisciplinary clinicians such as physiotherapists to deliver ongoing clinical activities, rehabilitation, recovery and support with daily routines and personal care.

Responsibilities

  • Undertake a range of delegated clinical, therapy and nursing led activities to support service users on the Integrated Network Team caseload.
  • Provide safe, patient‑centred and effective clinical activities in the community setting.
  • Work within adults’ services to deliver a full range of healthcare activities.
  • Collaborate flexibly with other teams or services, both within the locality and Sirona wide, when the need arises at short notice.
  • Travel throughout the service delivery geographical area.
