An exciting opportunity has arisen for a Band 3 Nursing Support Worker to join the community Heart Failure Service in Pembrokeshire.
This role is varied and will support both the clinical and administrative functions of the team. The successful candidate will work closely with the Heart Failure Clinical Nurse Specialists to support the delivery of high‑quality, patient‑centred care across community and clinic settings.
The post involves supporting nurse‑led assessment clinics, assisting with home visits, and undertaking delegated clinical tasks such as ECGs, venepuncture and observations, alongside a range of administrative duties to ensure the effective running of the service.
We are looking for a motivated, organised individual with good communication skills who is able to work flexibly as part of a team and use their own initiative.
Main duties of the job
- Support the Heart Failure CNS team in delivering care to patients in clinic and community settings
- Assist in nurse‑led assessment clinics, helping prepare equipment, manage patient flow and support clinical assessments
- Accompany nurses on home visits where appropriate
- Undertake delegated clinical tasks such as ECG recording, venepuncture and monitoring of observations, in line with training and competence
- Recognise and report any concerns or changes in a patient’s condition promptly to a registered nurse
- Provide reassurance and support to patients and carers, promoting a positive patient experience
- Carry out a range of administrative duties, including booking appointments, managing clinic lists, data entry, and handling telephone enquiries
- Maintain accurate and timely documentation, ensuring confidentiality is upheld at all times
- Support the smooth day‑to‑day organisation of the service, prioritising tasks appropriately
- Work collaboratively with the wider multidisciplinary team
Person Specification
Qualifications and Knowledge
- Level 3 qualification in a health‑care related subject or equivalent experience, with evidence of work‑based competency demonstrating theoretical knowledge and ability to undertake clinical care duties relevant for the role.
- Knowledge of the Code of Conduct for Healthcare Support Workers in Wales and All Wales Delegation Guidelines (HEIW).
- Knowledge of relevant policies and procedures within a healthcare setting, including safeguarding adults/children, confidentiality, and GDPR.
Experience
- Experience of working within a relevant health or social care environment, providing direct patient care.
- Experience of working with patient groups relevant to the area of practice.
- Experience of working within the NHS.
Language Skills
- Welsh speaker (Level 1). The ability to speak Welsh is desirable; English and/or Welsh speakers are equally welcome to apply.

Salary
£26,813 to £27,890 a year pro rata.
