Social Worker, 0–25 Disabilities: Family-Centered Care & Transitions

London Borough of Lambeth is seeking a qualified social worker to join the 0-25 Disabilities Service on a full-time fixed-term secondment for 12 months. You will work with children and young people with complex disabilities to assess needs and plan ongoing support.

The role covers safeguarding, care planning, and joint work with families, education, health, and partner agencies to promote independence, inclusion, and wellbeing. Salary ranges from £44,937 to £51,228 per annum.
