Community Mental Health Support Worker (Discharge)

Venture-People is recruiting a Mental Health Support Worker for our DORS Team in West Sussex. Based in our discharge service, you will work with a multidisciplinary team to support individuals in the first six weeks post-discharge, helping reduce readmissions and promote recovery in the community.
