Shifts: 7‑day week rota; shift pattern can be 3 or 4 long days or 5 shorter days; block booking only.

Key Responsibilities

• Deliver high‑quality nursing care in the community, empowering patients to maintain independence.
• Assess, plan, implement, and evaluate person‑centred care plans.
• Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
• Assist in facilitating smooth hospital discharges and reducing readmissions.
• Perform clinical skills including:
  • Complex wound care including Doppler compression
  • Simple wound care
  • End‑of‑life care
  • Medication administration
  • Catheterisation (male, female, suprapubic)
  • Bowel care
  • Syringe driver
  • Insulin administration
  • Doppler assessment
  • Compression bandaging

Essential Qualifications

• NMC registration (valid and active).
• A minimum of 6 months NHS experience as a Community Nurse.
• Eligibility to work in the UK (no consideration for applicants without valid UK work eligibility).
