Job Description:

This role involves enabling choice, advocating for individuals in multi-disciplinary meetings, and knowing when to escape concerns.

You’ll promote local resources and well-being opportunities, helping people advocate for themselves. You’ll develop personalized learning and well-being plans, focusing on safety and utilizing community resources. Support will be provided through various communication methods, ensuring confidentiality and effective care planning.

By addressing health inequalities and engaging communities, you’ll build essential partnerships with local professionals. Accurate record-keeping and effective caseload management are key.
