Position Requirements

• MD/DO Degree.
• Board certified.
• Florida medical license.
• DEA license.

Required Qualifications

• Licensed to practice medicine in accordance with Chapter 458, Florida Statutes plus four years of appropriate experience. Completion of a residency program may substitute for the required experience.
• Federal DEA certification.

Preferred

• Specialty license or certification.
• Board certified or board eligible in specified area of medical specialty.
• Certification/ licensure in the state of Florida and federal DEA certification.
