What you will do:

  • Conduct comprehensive assessments within the community for individuals with permanent or complex disabilities, ensuring the provision of services that prevent ongoing dependency.
  • Perform assessments in accordance with relevant legislation including the Care Act and Housing Grants, Construction and Regeneration Act, to reduce handoffs while ensuring environmental changes bolster independence.
  • Collaborate within a multidisciplinary team to appraise the effectiveness of current care provisions, finding innovative ways to promote self-reliance among service users.
  • Oversee and guide the work of Occupational Therapy Assistants and students, fostering a culture of learning and improvement.
  • Participate in initiatives within the OT and Sensory service that advance the aspirations and vision of Adult Social Care.
  • Engage constructively with service users, carers, and other stakeholders to empower individuals, maximising their independence.

In this role, you must be HCPC registered and a confident car driver to facilitate community visits.
