Recovery Coordinator & Case Manager (Substance Use)

Change Grow Live in Scotland is recruiting a Recovery Coordinator to assess and engage with service users, ensuring access to the right intervention at the right time within the Forth Valley Recovery Service.

You will manage care and recovery plans, provide screening and referral, support social reintegration, and work with partners to deliver group content and positive outcomes for individuals on their recovery journey.
