Leicestershire County Council is seeking Community Support Workers to join the Home First Service across Hospital Discharge, Central Duty and Early Review teams. The role supports timely hospital discharge and home-based care, with a pathway from Grade 8 to Grade 9 after training and probation.
A car is essential for some posts and flexible/hybrid working may be available. Vacancies include Early Review, Brokerage and Discharge Hub teams at County Hall and associated sites.
