A well-established community services provider, known for its comprehensive range of social welfare services, is actively seeking a dynamic and highly skilled individual to fill the role of Home Ageing Services Coordinator. Join the committed team in delivering high-quality services to seniors facing challenges associated with ageing, including dementia, chronic diseases, and mobility issues.

Position Summary:

As the Home Ageing Services Coordinator, you will play a vital role in supporting the Executive Team of the Home Ageing Services (HAS) Unit. Your primary responsibilities include developing and delivering top-notch services to seniors, ensuring alignment with established service guidelines, organizational policies, and Work Health and Safety (WHS) procedures.

Main Duties and Responsibilities:

  • Provide case management to home care clients, conducting assessments, and developing individualised support plans.
  • Actively promote and expand home care services, taking the lead in engaging potential clients.
  • Compile statistical data for efficient management of Home Care Services, update client details, and maintain the database.
  • Ensure services comply with funding requirements, program guidelines, Aged Care Quality Standards, and all relevant legislation.
  • Manage funding applications and projects related to care services, coordinating activities, resources, and ensuring timely completion.
  • Actively participate in ongoing quality improvement activities.
  • Fulfil any other duties as required by the Executive Team of the HAS Unit.

Requirements

  • Tertiary qualification in nursing, health, welfare, or related areas.
  • Minimum 2 years’ demonstrated experience in Government-funded home-aged care programs.
  • Knowledge and experience applying a person-centred approach and consumer-directed care model.
  • Strong ability in developing and implementing quality improvement measures.
  • Excellent spoken and written English, communication, interpersonal, time management, and computer skills.
  • Experience working with primary and allied health in providing services to clients.
  • Current First Aid Certificate.
  • Valid police check certificate or ability to obtain prior to employment.
  • Valid Australian Driver’s Licence with access to own vehicle.
  • Completion of at least two doses of COVID-19 Vaccination.
  • Experience in case management, marketing, and business management.
  • Demonstrated experience working within financial budgets.
  • Strong supervisory and organizational skills.
  • Ability to prepare funding submissions and reports independently.
  • Supervision and mentoring skills with experience in staff management.
  • As per Enterprise Agreement.
