HIV Re-engagement CNS – Community Outreach (Band 7)

Chelsea and Westminster Hospital NHS Foundation Trust in London is recruiting an Outreach HIV Clinical Nurse Specialist (Band 7) to enhance re-engagement in HIV care among people out of care, delivering a hospital-without-walls approach across community settings.

You will work with MDT colleagues, peer navigators and VCSO partners to identify, trace, and re-engage individuals, undertake holistic assessments, initiate ART where appropriate, and support retention in care.
