Community Health Nurse – Home Visits (Band 5)

Cheshire & Wirral Partnership NHS Foundation Trust is seeking a Band 5 Community Staff Nurse to join the Chester South Care Community Team. You will deliver a range of quality nursing services and holistic assessments to patients in their homes, supported by a multidisciplinary team.
