Temporary until April 2027

This is an exciting time to join our Integrated Cluster Network Teams. The Cluster Team work and support to develop a team around the person and their family or those important to them. The people we support can have long term complex challenges around their physical health, mental health and emotional well-being. It works with both older and younger adults. We aim to provide holistic support to allow them to live their lives as they wish to and ensure voice, choice, independence and control in their lives. The Teams consist of Social Workers, District Nurses, Occupational Therapists, Physiotherapists, Dietitians, Speech and Language Therapists, Pharmacy and Community Psychiatric Nursing. We work closely with GP surgeries and third sector partner agencies to assess and support the needs of adults affected by long‑term complex and adverse conditions as well as their carers and families.

Key Responsibilities

  • Promote and deliver high standards of professional practice, intervention and social work leadership within the Integrated Cluster Network Team.
  • Support the Social Work Team Manager and other multi‑professional team members, providing high‑level assistance.
  • Lead and mentor the social work team, inspiring and promoting the delivery of the highest level of practice skills and developments.
  • Oversee interventions in a dynamic multi‑disciplinary team and provide social work leadership and supervision.
  • Ensure an integrated approach to care and support, focusing on the person’s voice and the needs of families, carers and those important to them.

Qualifications and Requirements

  • Experience as a professional social worker and ability to lead within an integrated service.
  • Knowledge of the legislative framework and innovative, creative approaches to social work.
  • Ability to greet customers in Welsh – speaking Welsh is a requirement.
  • Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) required.
  • Valid driving licence.
  • Protecting children, young people or adults at risk is a core responsibility and must be upheld.
  • Will work in accordance with the council’s Hybrid Working Policy, balancing home and office hours.
  • Relocation package up to £8,000 may be considered.
