Job Overview

Post holder is responsible for ensuring care to the relevant client group who require Community liver health checks and liver cancer surveillance within Lancashire and South Cumbria Operational Delivery Network. East Lancashire Hospitals NHS trust is the host site and work base. The post holder supports and assists the clinical teams in four provider trusts, and other partner organisations. Liaison with primary care will also be essential as part of the ongoing national pilot to support case finding.

The post holder acts as a positive role model within the network for staff, patients, families, carers, and works closely with the ODN delivery group and provider trust clinical teams to provide the highest standards of care to patients. There is a requirement to contribute to data collection for NHSE liver surveillance data and the community liver health check pilot.

Key Responsibilities

  • Diagnose, assess and care patients at risk of liver disease.
  • Find and recruit patients, conduct testing and liver assessment.
  • Collect data for NHSE liver surveillance and pilot programmes.
  • Support the ODN in delivering community liver health checks, ongoing assessments (diagnosis) and enrolment onto liver surveillance.
  • Monitor, support and diagnose defined patients within clinical protocols and guidelines.
  • Provide liaison with primary care and partner organisations such as drug and alcohol services, custodial premises, hostels, homeless centres, prisons and probation services.

Qualifications & Development

We would consider a band 5 looking for a development opportunity or secondment (if approved by manager). Full training and support will be provided.
